HDHS
HDHS supports a continuum of care model that encompasses the journey of life through the coordination / provision of health promotion, early intervention, diagnosis, treatment, rehabilitation and palliation. All health professionals work with the community, family and individuals to provide continuity of care.

HDHS is building a service model, which places an emphasis on providing services that reflect the demand for health and health care within the Hawkesbury Local Government Area. This is being achieved via a collaborative planning approach with other local human care agencies. This facilitates a continuing process of consumer participation at all levels of the continuum from services planning through to prevention and early intervention strategies with joint responsibility in the management of their treatment and rehabilitation processes.

There are currently three program streams within this project. They are:

  • child and family health
  • youth health
  • aged care

All streams are accompanied by special projects have coordinators responsible for them and are linked to the health services strategic plan.
